ABSTRACT
There is no central data collection system for the collation of human error incidents in the ESI and therefore no national details or aggregated data are available for analysis or investigation. Few studies exist in this area and no recent systematic study has been undertaken of incidents in this field. An earlier study is that of Hale (1970).
