ABSTRACT

A 9-year-old boy, with no significant past medical history, is referred to your clinic for a 1-year history of intermittent abdominal pain. The episodes were rare and mild at the beginning. Over the last 6 months, they have increased in frequency and severity and are typically accompanied by vomiting. The emesis often consists of previous meals but has been dark green on a number of occasions, including during the week prior to the clinic visit. Over the past 3 months, his stool has been found to be frequently loose, foul-smelling, and floating in the toilet. A proton-pump inhibitor prescribed by his pediatrician has not improved the symptoms.
