ABSTRACT
Safe and efficient operation of oil refineries is critical to the US economy. However, fatalities and disruptions in production continue to occur as a result of human factors deficiencies. This paper outlines the history of human factors issues in refinery operation, including progress to date on improvement. Key issues and opportunities will be highlighted. The discussion is oriented toward the human factors professional that may not be aware of refinery operational issues.
