ABSTRACT

In its 2001 study, “Crossing the Quality Chasm: A New System for the 21st Century,” the Institute of Medicine decried the absence of “real progress towards solving these problems” because care delivery continues to rely on “…outmoded systems of work. Poor designs set the workforce up to fail, regardless of how hard they try. If we want safer, higher-quality care, we will need tohave redesigned systems of care” (Institute of Medicine, 2001).
