As this chapter is written, most physician practices still capture patient health information on paper. Aspatientsenteraphysicianofficeforavisit,aclipboardfullofregistrationformsalongwithapenis placedintheirhandsbythereceptionist.Patientsthenstruggletocompletethoseforms,tryingtorecall thenamesandproperspellingsofmedications,whentheyhadtheirlasttetanusshot,whatyeartheyhad hernia surgery, and the phone number for their primary care physician.
