ABSTRACT
It is understood throughout the safety profession that the most important role a safety professional plays is the prevention of accidents and injuries. There have been varied and differing discussions as to whether accidents are totally preventable. It is my personal belief that all accidents are preventable. However, when an accident or injury occurs, it is the responsibility of the safety professional to ensure that a thorough investigation is conducted. This gives rise to the question, “Why do we conduct accident investigations?”
