ABSTRACT
Introduction So far we have emphasized hospitals, nursing homes (skilled and unskilled facilities), clinics, and urgent care facilities. We have mentioned the need for improvement in the physician’s practice (Chapter 2), but we have not made it a strong point. In this chapter, we are going to discuss a typical physician’s practice and a patient-centered medical home. Our discussion is going to focus on the initiatives that may be taken for improvements. We are also going to give a very detailed example of how a primary care practice should start to focus on its processes.
