Information sharing about the factors that cause trouble and human error

In recent years, strategies for reducing accidents have shifted from “prevention of recurrence” to “prevention of occurrence.” The concept of “prevention of occurrence” can be traced back to Heinrich’s Law (1931) (Figure 1): “for one fatal or serious accident, there are 29 accidents and 300 potential incidents with a high possibility of injuries.” Hence, it is important to take measures against potential incidents to prevent serious accidents.
