First, let me admit to how impossible this expectation is for operators in a Stage 1 plant. They’re likely the subject of disciplinary action if they have repeated cases of running material that is not per specification or if there is recurring evidence of product they ran being rejected or causing scrap downstream in the process. Sure, sometimes operators are careless and may need a wake-up call, but this is the rare exception. If you were constantly being chastised for having quality issues, though, would you continue to write the reject tags or otherwise draw attention to yourself? Human nature says we all would be more likely to let some things slide.
