ABSTRACT
Introduction Even a super•cial review of the history of major accidents makes it clear that the precursors to disaster are not hidden-they are generally well known within an organisation, or parts of it. Major inquiries and legal proceedings all too often identify that clear warning signs about factors that cause or contribute to major accidents existed prior to the accident, and in many cases were brought to the attention of relevant managers.
