ABSTRACT
Introduction If there is one constant in modern workplaces, it is change-a lot of it and the potential for it to occur very quickly. When change happens, the systems that have been put in place to manage workplace safety risks may very quickly become obsolete. Indeed, they may increase the risk of a workplace accident.
