ABSTRACT
Introduction One of the dominant themes that emerge in most major accident inquiries is “production before safety”. Production before safety manifests itself in many different ways: from accepting the cheapest bidder, to shaving precious minutes off schedules and deadlines; from choosing less expensive equipment to accepting the way that work has been completed and not being prepared to spend the time to go back and check that it was completed safely.
