ABSTRACT
Introduction Safety rules-processes, procedures standards and so forth-are one of the most ubiquitous features of modern safety management. They dominate the safety landscape. The suitability of safety rules and rule compliance are always a key and critical issue in a major accident inquiry. The development of more or new rules is a typical response to safety system failure.
