ABSTRACT

Nutrition problems of patients with chronic kidney disease (CKD) should be addressed in all medical care settings. Nutrition screening and assessment are the building blocks of planning nutrition intervention for patients with CKD. Nutrition care process (NCP) starts with screening by a quali-”ed health-care provider who will identify individuals at nutrition risk who have the need for a more detailed assessment. If the screening instrument’s criteria indicate the need for a detailed assessment, then a comprehensive nutrition evaluation will follow.
