ABSTRACT

The contentious title for this chapter has been chosen because these words are important for many people involved with this topic. It is understandable that we seek to assign blame when waste and damage occur. Often we do this at the point where waste and damage have occurred already, and, for a variety of reasons, we look back to assign blame. The purpose of assigning blame might be to locate the blame away from ourselves, or it might be that while we are not involved ourselves we might seek to learn from the situation in order to prevent it happening again. Alternatively we may want to fire someone! Whatever the reason, the apportioning of blame can be an emotional process, and we should be aware of this from the start.
