We all make mistakes. We all recognize when a defect occurs caused by a mistake or by an error.
For example, incorrect information on documents, such as an invoice with incorrect address, name, amount, or quantity, is due to error. It need not be a person who made the error. A machine might have a software miscoding that searches for the product code to determine the invoice amount.
