Myocardial necrosis shows delayed gadolinium (Gd) enhancement, but viable myocardium does not.
The transmural extent of enhancement predicts the likelihood of functional recovery following revascularization: • < 25% transmural extent: high likelihood of functional
improvement • >75% transmural extent: low likelihood of functional
improvement
Susceptibility artifact can mimic a subendocardial defect on adenosine perfusion imaging. In favour of artefact are that its appearance coincides exactly with arrival of the Gd bolus in the left ventricular cavity. A true perfusion defect tends to appear slightly later.
