Indications for radiotherapy Primary carcinomas of the vagina are rare, with 80 per cent occurring in women over 60 years as SCCs. Clear cell adenocarcinomas of the vagina are seen in young women who are exposed to oestrogen therapy in utero. More commonly, tumours found in the vagina are an extension from primary cervical, vulval or urethral tumours, local spread from the rectum or bladder or metastatic lymphatic or vascular deposits from endometrial, breast or lung carcinomas. Surgery is the mainstay of treatment for malignant melanomas of the vagina, which have a high incidence of distant metastases and poor overall survival rates.
