Muscle imbalance In neurogenic pes cavus there may be muscle imbalance at the ankle, the subtalar and midtarsal joints and at the joints of the toes.2 At the ankle there may be weakness of either the dorsiflexors or the plantarflexors. At subtalar and midtarsal joints there may be an imbalance between the invertors and the evertors of the foot. At the toes there may be an imbalance between the flexors and extensors of the metatarsophalangeal joint and the interphalangeal joints on account of intrinsic muscle paralysis.
