A 27-year-old woman, who has recently returned from holiday in Africa, presents to accident and emergency with a 7-day history of fevers, sweats, headache, malaise and lethargy. On examination, her temperature is 39°C. Cardio respiratory and gastrointestinal examinations are unremarkable. What is the most likely differential diagnosis?
