ABSTRACT

History A 66-year-old man presents to his GP complaining of a 3-week history of progressive leg swelling. He has also noticed that his urine has become more frothy. There is no history of shortness of breath or chest pain. He has a past medical history of mild asthma for which he takes a salbutamol inhaler as required. Four weeks ago, he had sustained a back injury for which he had taken over-the-counter ibuprofen for 1 week. He is a non-smoker and drinks 18 units of alcohol per week.
