ABSTRACT

History A 70-year-old woman is referred to the acute medical unit with a 3-day history of fever, mild confusion and urinary frequency. Her abbreviated mental test score is 7/10. She is a recently diagnosed type 2 diabetic for which she takes metformin 500 mg three times a day. She has a past medical history of hypertension and hypothyroidism. Her other medication includes lisinopril 5 mg/day and levothyroxine 75 μg/day. She has no allergies. She lives with her husband and is totally independent. She is a non-smoker and drinks 30 units per week.
