ABSTRACT

Studies show that 90% to 95% of workplace incidents are avoidable and 80% to 85% are repeated. Learning from events is an effective means for reducing repeat incidents and cost while improving operating reliability and performance. Learning from incidents and knowledge generated from best practices and continuous improvements seldom make it to the frontline where such learning can be bene–cial in preventing incidents. More important, Shared Learning is the last remaining low hanging fruit for sustained improvements in reliability, reducing incidents in the workplace, and improving organizational performance.
