ABSTRACT
Knowing which elements to “string together” is key to painting the right picture to root out the problems. Hospitals that successfully convert multiple data sets into actionable intelligence can quickly implement strategies for change. The following steps will take readers through a data analysis process that will highlight the role of the discharge planning department and its relationship to driving hospital outcomes or COS-Q (cost, operations, service, and quality of care).
