ABSTRACT

Patient safety is an integral component of STEEEP care. As defined by the Institute of Medicine (IOM), safe care should avoid injuries to patients from care that is intended to help them.1 Consistent with this definition, the BHCS patient safety vision has three components: (1) achieving no preventable deaths, (2) ensuring no preventable injuries, and (3) seeking no preventable risk.2,3 These goals require strategic efforts in the categories of culture, processes, and technology.
