Concept of stress myocardial imaging Myocardial blood fl ow distal to coronary artery stenosis is normal at rest unless occlusion is greater than 90% of luminal diameter, or a myocardial infarct is present. However, following maximal stress, the normal increase in myocardial blood fl ow becomes impaired distal to stenosis of approximately 45-50%, or greater.
