ABSTRACT

Additional comments There is inferior ST segment elevation with reciprocal lateral ST segment depression. The right-sided chest leads show ST segment elevation in leads V3R-V6R.

1. This is an ECG showing the usual limb leads but right-sided chest leads (V1R-V6R). Perform an ECG with right-sided chest leads in all patients presenting with an acute inferior myocardial infarction, to look for evidence of right ventricular involvement (as shown by ST segment elevation in lead V4R).
