There are also disciplinary and juridical reasons for the investigation of accidents. We are here considering rule violations and criminal negligence. It is the duty of the workers to follow the work and safety rules. An accident investigation will often reveal violations of such rules. We also have to consider the legal consequences of the fact that safety is a line-management responsibility. Supervisors and managers at higher levels may be held accountable for the accident in cases where the investigation reveals substandard practices or conditions for which management is responsible.
