Indications Insulinoma is the most common neoplasm of the endocrine pancreas. They are

found with an equal distribution within the head, body, and tail of the gland. Since 90% of insulinomas are solitary, benign, and measure <2 cm, enucleation is an ap­ propriate treatment strategy as long as there is no evidence of malignancy such as invasion into surrounding structures or metastatic disease. Moreover, those tumors that are located deep within the gland or intimately related to the main pancreatic duct on imaging are not amenable to enucleation.
