ABSTRACT
Operator error is inevitable in systems of any design. The operators, like any other component in a process plant, have a certain capacity. When demands on the operators exceed their capacity, the result will be a failure or an error. The operators also inevitably have a stochastic aspect to their behaviour. If the system cannot tolerate the occasional randomness in their responses, the result will be an error.
