ABSTRACT
The frequency of errors in written procedures depends, of course, on the quality of work that goes into them. Various approaches were observed in new plants:
• Preparation by experienced operation engineers • Preparation by design engineers • Preparation outsourced to low-cost contractors, with checking by experi-
enced operators
Of these, only the first was found to provide good results from the start, but good procedures generally involve considerable review by experienced operators.
