ABSTRACT
An example of complete hazard identification is made here, to illustrate the principles from earlier chapters. The result is shown as a cause-consequence diagram in Figure 20.1. The identification, shown in Figure 20.1, is based on analysis of all steps in the procedure, using analysis sheets like those shown in Figure 18.1. Both possible human errors and latent hazards are identified.
