ABSTRACT

A 28-year-old patient seeks medical care for evaluation because he is worried about his health. The patient has no complaints, does not report any diseases or the chronic use of medications. His father died of acute myocardial infarction at 54 years of age. The patient eats his meals outside the home and his diet is poorly fractionated, poor in fruits and vegetables, rich in saturated fatty acids and with an energy content exceeding ideal levels. Physical examination reveals blood pressure of 134 × 88 mmHg, abdominal circumference of 110 cm and acanthosis nigricans in the occipital region. He reports that his weight was 79 kg 10 years ago and now is about 98 kg. The patient returns 1 week later with the following blood test results: fasting glycemia = 110 mg/dL, triglycerides = 280 mg/dL, HDL-cholesterol = 30 mg/dL.
