ABSTRACT
The works of Karl Weick, Kathleen Sutcliffe, and James Reason are popular in healthcare safety practices. I united their thoughts into one diagram.1,2 In their books on reliability in safety, they share similar concepts of the subcultures making up a culture where safety is achieved. In no way are these a dichotomy as you see the comparison that is diagrammed in Table 4.1.
