ABSTRACT
ACOs have the potential to lower costs, but financing accountable care is challenging in a health care system that has typically focused on treatment rather than prevention and has been shaped by the fee-for-service payment model. Factors like evolving technology, poor coordination between unconnected care sites, poor end-of-life care, and an aging population more susceptible to chronic diseases have further driven costs to unsustainable levels (Chapter 2).
