ABSTRACT
In the past, accidents occurring in the industrial sector were reported generally in terms of technological failures, and the human element in the accident causation tended to be overlooked. Nowadays, the role of human factors has become more apparent, since the frequency of technologyrelated malfunctions has diminished quite considerably. Over the years, human factors and fatalities have become an important issue in the oil and gas industry.
