ABSTRACT
I understand that I am using ultraviolet (UV) type B therapy for my skin disease. I understand that I must take the following precautions:
(1) I will wear the specified UV protectant goggles during treatment. Failure to wear protective goggles may result in severe burns or long-term injury to eyes.
