12.9.1 DVT prophylaxis The incidence of DVT in general surgery is approximately 20-70%. Recommended prophylaxis is tailored to the risk classification (see Table 12.2). General surgery patients at low risk do not require prophylaxis, except for early ambulation. Lowdose heparin (5000 IU SC every 12 h), LMWH, intermittent pneumatic compression sleeves or gradient elastic stockings are to be given to moderate-risk surgical patients. In high-risk general surgery, low-dose heparin (5000 IU SC every 8 or 12 h) or LMWH is recommended. Placement of a vena caval filter as prophylaxis for major surgery is recommended in patients with significant cardiopulmonary disease and in patients who cannot receive any form of prophylaxis following high-risk surgical procedures.
