ABSTRACT

This is a traction apophysitis of the epiphysis at the insertion of the patellar tendon into the tibial tuberosity. The peak incidence occurs between the ages of 10-14 years. The patient is usually an active boy who complains of localized pain in the anterior aspect of the knee. Examination reveals a prominent tibial tuberosity with localized tenderness. This is a self-limiting condition. Acute exacerbations are treated with rest and NSAIDs. There is no need to advise long-term restriction of activity if the discomfort is tolerable, merely short periods of rest during acute exacerbations. Reassurance should be given that the condition will settle spontaneously as the epiphysis fuses. In severe cases the knee can be immobilized in plaster for several weeks to enforce rest.
