ABSTRACT

In this and the following six chapters we describe in detail the application of CBT for CH to a range of individuals suffering from command hallucinations, starting with Tom. In all cases the therapist was Dr Sarah Byrne, the first author. The cases have been chosen from the 19 participants in our trial (Chapter 11) to represent the wide range of types and levels of difficulty experienced by men and women from differing backgrounds and ages. Thus we have not selected our ‘best’ cases but a range from the more to the less successful, in order to provide the clinician with a realistic picture of the challenges involved in the application of CBT for CH. The presentation of each case follows the protocol described in Chapter 3, although, as the reader will see, this is done with considerable flexibility and care, to accommodate to the idiosyncratic needs of the individuals. The reports begin with some brief background information about the individual, followed by the main sections of assessment and intervention (subdivided to describe the key tasks and issues addressed), and concluding with an account of the outcome, with data from the key measures showing the progress (in most cases) of each individual. Recommendations for continuing care are also included in every case. We begin with Tom.
