ABSTRACT

Sometimes you need to note a student’s progress or document when a problem is indicated. Such notes are essential when it comes to assessing academic or behavioral progress, communicating with parents, and keeping you in tune with a student’s needs. An easy way to take these notes and compile them is to use a note folder. Simply tape 5 × 7” index cards in a stair-step fashion on one side of the folder, and then place the students’ names alphabetically on the steps. When you see something noteworthy, jot it down on the card with the day’s date. This note file helps you maintain formative assessment information all in one place. It also allows you to quickly scan cards to determine whether the same problem is occurring in many students, in which case you need to reteach the concepts.
