Varied approaches can be taken to record keeping, although this is not one of many counselors’ favorite activities. One problem may be that few people take the time to design and maintain a satisfying system. Organizations who conduct peer review or accredit counseling agencies usually require that records be consistent, clear, continuous, and complete. For a sexuality counselor to try to “keep everything in my head” would seem arrogant, because the details of treatment plans and client response are so individual. On the other hand, voluminous records waste valuable professional time. What is the happy median?
