ABSTRACT

As described in the rst paragraph by Dismukes (chap. 19, this volume), a for-gotten intention had drastic and tragic consequences for Flight 1141. Without a doubt, all of the crew members, engineers, pilots, and air trafc controllers shared a common goal: to make sure that the plane ew safely, ensuring the health and welfare of all passengers. However, one intention in a series of intentions meant to fulll that goal, namely to prepare the plane for takeoff, was forgotten.
