ABSTRACT
Bob was referred to the home health care agency from his medical clinic. He had been released from prison several months before. Prison hospital records showed that he had a low CD4+ count and a high viral load. Since leaving prison, he had been uncooperative with clinic appointments. Because of his high risk for opportunistic infections and disease progression, the physician's request was to discover why he did not continue to seek medical care on a regular basis.
