This chapter originated as a study commissioned by the Nathan Cummings Foundation in 2001. The conclusions in the original paper were addressed to a relatively narrow circle of readers within the philanthropic profession; they have been lightly rewritten to interest a broader readership, and a few points have been selectively updated.
