One-third of the English population have a long-term condition, and a similar number of those have a mental health problem. The Covid epidemic could worsen this, but this is uncertain at the time of writing. Core CBT skills can be used in assessing and working with this group, and there is a desire to do this. The main patient groups are those with long-term conditions, medically unexplained symptoms and health anxiety. Therapists should ensure that practical adaptations are made, that therapist can access health records, and extra focus is put on engagement with the patient and family. CBT protocols can be used for depression and anxiety and long-term conditions, but idiosyncratic formulations are best with MUS.
