Fall prevention and risk interventions that are implemented should be documented in the electronic medical record or an appropriate downtime form. From the medical and surgical floor to the emergency department and the outpatient departments, the risk of fall is always around the corner. Universal fall prevention interventions/low-risk interventions include a full assessment of the patient’s coordination or balance before assisting with transfer/mobility activities. Staff need to provide an age-appropriate explanation regarding the fall precautions and educate patients and family/significant others about their responsibility in preventing falls and calling for the team when needed. If the fall occurs, it is imperative to have a protocol and to provide care as the patient’s condition requires. Initiate appropriate fall risk interventions based on assessment and a risk assessment to avoid possible recurrences. Nursing documentation should include document assessment including patient/family education of the risk of fall.
