ABSTRACT
A process for developing effective questionnaires is described. The process includes initial considerations, clarification of concepts, determination of question types, format and sequencing, pretesting, pilot testing, and administration. Open-ended and closed-ended questions are defined and explained. A process for developing good questions is described, as well as how to format effective questionnaires.
