ABSTRACT

This chapter introduces the concept of changing the way healthcare changes through transformation and population-health management. It discusses J. P. Kotter's original Change Management Model and an update and explores the difference in change management and transformational change. The chapter describes the strategic imperatives for population health and explains the technology enabled population segments. The Office of the National Coordinator for Healthcare Information Technology encouraged the use of Kotter's model in their 2016 primer, Change Management in electronic health record Implementation. Connected health services are expected to dramatically transform clinician-to-clinician, provider-to-patient, and consumer-driven interactions via technology-enabled consultation, direct patient care, self-management, and educational services. Transformation and innovation will become increasingly possible as healthcare industry transitions from “keep the lights on” information technology activities to focus on complex change initiatives needed to support population health management and the assumption of risks.
