ABSTRACT

This chapter describes the differences in brick-and-mortar, transitional, and digital health, connected health stages in the transition to value-based care. It explores the integrated roles of IT, Informatics, Analytics, and Quality in the virtuous cycle of value management. The chapter provides the several layers of the Care Management Platform and its role in preparing for population health and the assumption of risks and explains the Leadership models for each stage of the transition. The healthcare industry has begun the shift to high-value healthcare through a variety of new incentive programs focused on reimbursing providers based on the quality rather than the quantity of care they give to their patients. While the pursuit of high-value healthcare is a common theme for health systems, physician groups, and payers alike, the degree of change is unique for each market, health system, payer, and individual provider.
